REGULATIONS OF

THE „PROsenior" PROGRAMME

(hereinafter referred to as „the Regulations")

§ 1.

GENERAL DECISIONS

1.The initiator and organizer of the PROsenior Programme (hereinafter referred to as "the Programme") is the „PROJAN" Waksmundzki Family Foundation with the registered office in Warsaw, 02-956, 20 Resorowa St, NIP: 5213524250, Regon: 141802032, enrolled to the associations register, other social and professional organizations, foundations and the wholesome care public institutions of The National Judicial Register led by The District Court for the Capital City of Warsaw in Warsaw, XIII The Economic Department, in the KRS number 0000326507, (hereinafter referred to as "the Foundation").

2. The idea of The programme is to contribute to the thorough development of elderly and combating social exclusion, especially in such areas as education, culture, and also health protection, care and social aid. These actions support beneficiaries defined in 3 (1).

3. The programme was accepted to the realization on the strength of the Resolution of the Foundation Board No. 1 of August 31, 2010.

4. The programme shall be led on the territory of Republic of Poland in the first place, but can also be realized abroad.

5. The conditions of participation in the Programme are qualified in this Regulations, in the Foundation's Charter and in currently valid legal rules.

6. The participation in the Programme means the acceptance of the Programme principles enclosed in this Regulations and principles resulting from the other sources defined in (5) above.

7. Irrespective of decisions defined in (5) and (6) above, the participation in the Programme shall also be dependent on conditions defined in „Regulations of giving gratuitous services of social aid".

8. The Programme is organized from 31 August 2010 until recalled by the appropriate resolution of the Foundation's Board, with regard to the lack of damage for laws acquired by participants of the Programme.

9. The participation in the Programme is free. The Programme is funded from the Foundation's resources. Support is dependent on resources possessed by the Foundation.

§ 2.

WHAT DOES THE PROSENIOR PROGRAMME CONSIST IN?


1. The PROsenior Programme first of all consists of 3 projects:

a. Seniors and Culture

b. Seniors Know More

c. Seniors Going Out.

2. Every of above mentioned projects shall be realized once a quarter.

3. Within the project „Seniors and Culture" older persons are invited to places like museums, theatres, concerts and so on.

4. Within the project „Seniors Know More" older persons are invited to meetings with experts from various fields of life, e.g. with doctors, lawyers, scientists.

5. Within the project „Seniors Going Out" older persons are invited to Warsaw cafes' to common activities, such as painters' workshops or entertainment with logical games.

6. Within the PROsenior programme the Foundation also co-operates with the University of the Third Age of Szkoła Wyższa Przymierza Rodzin in Warsaw.The principles of co-operation are defined in the agreement drawn up between the Foundation and the School and also „Regulations of funding or financing activities of the University of the Third Age of Szkoła Wyższa Przymierza Rodzin in Warsaw".

7. There is also a possibility to support older persons in difficult life situation within the PROsenior programme. „Regulations of giving gratuitous services of social aid" is also applicable in this situation.

§3.

PARTICIPATION IN THE PROGRAMME

TO WHOM IS THE PROGRAMME DIRECTED?


1. Persons who came up to 60 shall participate in the projects of the Programme, defined in 2 (1) – (5).

2. Persons who fulfill conditions resulting from „Regulations of funding or financing activities of the University of the Third Age of Szkoła Wyższa Przymierza Rodzin in Warsaw" shall take part in activities of University of the Third Age. The necessary condition is to come up to 60 in this case.

3. Support, defined in 2 (7), shall be given to persons who fulfill conditions resulting from „Regulations of giving gratuitous services of social aid". The necessary condition is to come up to 60 in this case.

4. Duties resulting from the Regulations shall not apply to persons, who have participated in the Programme before the Regulations came into force.

WHAT SHALL BE DONE TO PARTICIPATE IN THE PROGRAMME?


5. A person interested in joining the Programme shall apply by making an appointment in the Foundation's registered office by phone (22 651 53 79 or 22 651 67 86).

6. During a meeting an interested person shall be asked to fulfill and sign the following documents:

a. personal inquiry / an application form,

b. statement concerning the part of the Programme in which that person would like to participate:

 in projects, defined in 2 (1)-(5);

 in activities of the University of the Third Age, defined in 2 (6);

 in the part, defined in 2 (7);

c. declaration of agreement on processing personal data,

d. declaration of agreement on using image (this document is not obligatory),

e. declaration of acquaintance with the Regulations,

f. defined in „Regulations of funding or financing activities of the University of the Third Age of Szkoła Wyższa Przymierza Rodzin in Warsaw" or „Regulations of giving gratuitous services of social aid", such as:

 income and permanent expenses confirmation,

 copies of documents confirming earnings and expenses,

 declaration that given information is true.

7. The Foundation reserves the right to request additional documents because of the specification of an application in special cases.

8. Persons giving untrue or incomplete data in filled out documents, shall be excluded from the Pogramme.

§4.

WHAT KIND OF SUPPORT CAN BE EXPECTED?


1. Participation in PROsenior's projects, defined in 2 (1)-(5).

2. Participation in activities of the University of the Third Age, defined in 2 (6).

3. Support concerning social aid, defined in 2 (7).

4. The Foundation reserves that forms of support, defined above, can be changed.

§5.

HOW IS THE SUPPORT GRANTED?


1. Fulfillment of duties, defined in 3, by an applicant, shall not yet be unambiguous with participation in the Programme.

2. Every request concerning participation in the Programme shall be verified individually.

3. The verification's procedure consists in analysis of the person's situation and making the decision, if the person qualifies to the participation in the Programme and in which of its part.

4. The Foundation shall not be connected with any deadline for verification of the application.

5. In case of making negative decision by the Foundation, it is possible for an applicant to apply for participating in the Programme again if the circumstances excluding from the Programme shall change.

6. An applicant applying for participating in the Programme shall be informed about positive decision made by the Foundation. Personal and contact data of an applicant shall be placed in the Programme's Participants base.

7. The Foundation reserves the right to support on altered principles, defined in the resolution of the Board relating to the concrete case.

§6.

RESTRICTIONS IN THE KIND OF SUPPORT


1. The Foundation shall not support repayments of debts, loans, credits.

2. The Foundation shall not give support in the form of loan.

3. The Foundation shall not give support for wide understood improvement of difficult life situation without indication of concrete aims or needs.

4. The Foundation shall not mediate in opening of subaccounts for private persons and institutions and shall not make accessible its bank account in order to accumulate beneficiary's means.

5. The Foundation shall not raise funds from payments of 1% of income tax.

§7.

TASKS AND RESPONSIBILITIES

OF PERSONS PARTICIPATING IN THE PROSENIOR PROGRAMME

a. The Foundation shall be informed about all telephone's and address' changes.

b. The Foundation shall be informed about the lack of possibility to participate in the event, organized by the Foundation, by the person who expressed the desire to participate in the event earlier.

c. Appropriate and cultural behavior on the meetings organized by the Foundation.

d. Reliable usage of given support.

e. The Foundation shall be immediately informed about the improvement of the family's financial condition.

f. Staying in the permanent contact with the Foundation.

g. Conscientious and honest performing duties resulting from participation in the Programme.

h. Making a statement, that the family-financial situation did not change, once every 6 months.

§8.

FINAL PROVISIONS

1. Detailed conditions of given support, in every concrete case, may result from the agreement

concluded between the Foundation and a person participating in the Programme, and also from the content of the Board's resolution approving support for the person.

2. The Foundation reserves the right to change the Regulations every time without giving a reason. Every change of the Regulations shall be made without damage for laws acquired by participants of the Programme.

3. The Regulations shall be accessible on the side of the Foundation at the address: www.frwp.pl and in the registered office of the Foundation. The Foundation shall send the Regulations by fax or by post on the interested person request.
